Complete this form to report a blood-borne exposure/injury OR any information regarding inappropriate/unwarranted events that have occurred while on your CBCE rotation.

Please reach out to Dr. Tomaka (satomaka) or Tracy Darnell (tdarnell) if you have any hesitations about completing this form (i.e., it's topic of a sensitive-nature).

Fill out the following link to request a change to your rotation; this must be done at least two weeks in advance of the change being requested

You are responsible for completing the following steps:

CALL THE CLINIC ASAP: All contact information is on our CBCE Website. Speak with the clinic as soon as you know you won't be present.

EMAIL CBCE: Send a message to Tracy Darnell (tdarnell) and cc Danielle Watts (wattsda)

COMPLETE A "REPORT AN ABSENCE" FORM: This form may be found on MiTools under the Student Tab.

**Reporting a last-minute absence is your responsibility. This process is not to be confused with requesting time off for other absences (interviews, etc.); those must still be done in advance.
