Credentialing Requirements

All outreach clinics require D4s to undergo a drug screening. Students will be contacted by Certiphi in advance of their rotation to complete this testing. Results will be sent to CBCE and shared with the student. Questions regarding these requirements should be sent to Dr. Mark Fitzgerald at markfitz@umich.edu.

All students should retrieve a copy of their original Background Check. In some cases, a copy of your original background check from your admission to the dental school will be required by the external clinic. If you don’t have a copy, please scroll to the bottom of this page for directions for how to retrieve your report; this report should not to be confused with the drug screen results mentioned above.

All students should retrieve a copy of their immunization records and a copy of their BLS/CPR card. Directions for retrieving these items are at the bottom of this page.

You must complete & submit all clinic-specific pre-rotation requirements by the due date included in your assignment email. Clinics will not let you practice if the paperwork has not been submitted.

IMPORTANT: Do not wait until the due date to submit your paperwork. If you are awaiting test results or other information, you should move forward with submitting any other required paperwork/forms in the meantime.

Retrieving Proof of Your BLS/CPR Certification

Claim your eCard via Email:

  1. You will receive an email from eCards@heart.org with a link inviting you to claim your eCard online. Please make sure to check your spam or junk folder. This link will direct you to the Student Profile webpage; please confirm that all information on that page (First Name, Last Name, Email, Phone Number (if entered for claiming by SMS), eCard Code, AHA Instructor Name, and Training Center information) is correct. If it is not, contact your Training Center.

  2. Set up your security question and answer. Accept the terms of the site and click “Submit.”

  3. Complete the survey on the course you just completed. Your answers to these questions will help improve the quality of future AHA training.

  4. After you complete the survey, your eCard will display. You can save or print your eCard.

  5. Once your eCard has been claimed, you will receive an email notification; please save this confirmation email for your records.

  6. Claiming your eCard is the only way you can show proof of course completion to your employer. If you need to email your eCard, please see step 5 under “How to View your Claimed eCard” (link to this answer when building the page).

Claim your eCard via eCards Site:

  1. Visit the eCards Search page. On the “Student” tab, enter your First Name, Last Name, and Email (ensure this is the email address that was used to sign up for the course) OR enter your eCard Code at the bottom of the page (ask your Training Center for this code). Click “Search.”

  2. If all the information was entered correctly, you will be brought to a page that shows “My eCards.” Look for your eCard that is “Unclaimed” and follow the steps through the site to claim your card, including setting up a security question and answer. If your eCard cannot be found, contact your Training Center to verify your information.

Retrieving A Copy of Your Background Check from Certiphi

This is NOT the same report as your Drug Screen performed at the end of your D3 year. This report was ordered by UM Dent Admissions prior to your first DDS year.

  1. Access the Certiphi Consumer Care website at https://consumercare.certiphi.com/.

  2. Click on “Don’t have your Case Report Number?”

  3. Complete the information required under “Identity Verification,” which includes:
    > your first/last name (at the time of your Dental School admission)
    > your Social Security #
    > your DOB
    > your zip code (at the time of your Dental School admission)
    Then click on “View Your Case Report”

  4. If your report is unable to be located, you should contact Certiphi Help Desk team at 1-800-803-7860 ext 2006 OR 1-888-291-1369, ext 2006.

  5. Again, this report was ordered prior to/upon your admission to the dental school and should not be confused with your drug screen that was ordered by CBCE.

Retrieving A Copy of Your Immunization Records

You may submit a request for your immunizations to the Dent-Registrar’s Office. Use this Universal Request Form (URF) and submit it to dentistry.registrar@umich.edu stating you need your immunization record.
